STUDENT ENROLLMENT

To enroll your child, you must live within the boundaries of the Curwensville Area School District. Please call Justine McGary at 814-236-1100 ext. 515 to schedule a time to pick up and drop off your packet.

In order to make the registration process as efficient as possible, you will need to bring with you to the Business Office located at 650 Beech Street Curwensville, PA 16833 the following: Completed Enrollment Packet, Proof of Date of Birth, Current Immunizations, Proof of Residency, and Guardianship & Custody Orders if this pertains to you.

Proof of student’s Date of Birth - original birth certificate, current passport, or baptismal certificate if the date of birth is shown.

Current Immunization Record – a list from your doctor is recommended, but can also be a list that you have kept since the student was a baby. Evidence of Immunizations is required by the Commonwealth of Pennsylvania.

Immunizations can be faxed to Attn: School Nurse at 814-236-7014

Proof of Residency – Any one of the following constitutes acceptable documentation:

1. A current deed;
2. A mortgage statement no more than three months old;
3. An active residential lease agreement;
4. A utility bill no more than three months old;
5. An internet bill no more than three months old;
6. A credit card bill no more than three months old;
7. A bank statement no more than three months old;
8. A tax bill no more than three months old;
9. An insurance document no more than three months old; or
10. A piece of mail sent in the past three months by the federal or state government such as benefits information, voter registration, or tax documents.

Guardianship & Custody Orders (if this pertains to you) - Persons with legal guardianship or legal custody of the student must bring a copy of the court document(s) granting the guardianship or custody.

Foster Students**- Letter of Placement by agency with required information pertaining to the placement.**

Parental Registration Statement**- (included in packet) Signed by parent or guardian.**

Students cannot be enrolled until we have everything listed above.
