The physical form (section 6) MUST be dated ON OR AFTER June 1, 2017. The forms, once completed, will be good for that sports season. Students participating in additional sports the following season(s) only need to complete the section 7 to participate in next sport.You may pick up physical forms in the Athletic Office or from your coach. The physical forms and athletic insurance statement form may also be accessed through the physical form and insurance statement tab found on the left side of this page.You may not begin practice until the physical/athletic insurance forms are complete including the medical form (section 6) signed and dated by a physician. It must be completed and signed by a Medical Doctor and cannot be a chiropractor.All Coach/Volunteers are reminded to complete the Concussion and the Cardiac trainings. Be sure to print certificate at the end of each training and turn into the Athletic Office. This must be done before any practices begin. Note: this is required to be retaken each school year. (For the links to these training click on the Concussion/Cardiac tab to the left of this screen.)All Coach/Volunteers are reminded to attend the Mandatory Child Abuse Reporting Training. Follow the Mandated Reporter Training link to the left and it will take you to the PA Child Welfare Resource Center.You will need to register and take the course. Upon completion you will need to print out the confirmation of viewing and turn that into the Athletic Office. This training is required every three years.As you are aware of, in July 1, 2016 all coaches and volunteers currently employed at a PIAA member school are required to complete a Basic Coaching Principals Continuing Education Course and also a CPR & First Aid Course.
