Summer Physicals will be done on: June 27 from 8:30am - 12noon and 1pm - 3pm. This date will be reserved for FOOTBALL unless you will not be able to come to the July physical. July 18 from 8:30 am - 12 noon. Half day ONLY... this date will be reserved for all other sports unless you were not able to come to the June 27th physical. PHYSICALS WILL BE DONE AT DR. HAYS' OFFICE. 617 State St. Curwensville Remember if you need a sport physical for 2012-13 school year it MUST be dated ON OR AFTER June 1, 2012. The form, once completed, will be good for the whole year. Pick up a physical form in the Athletic Office or from your coach. **Remember to turn in your completed physical for 10 days before your physical to the Athletic Office so we can give the dr. an accurate count of athletes.
